If you are interested in applying for admission to the graduate program in Applied Mathematics or Statistics, you may want to submit this questionnaire for an informal feedback about how well your educational background fits the demands of the program.

This form is not a substitute for the formal application. Its purpose is to provide only a quick preliminary feedback before you proceed with an application through formal channels. The department provides a limited number of financial aid packages in the form of Teaching and Research Assistantships. Typically these provide tuition for up to 10 credit-hours of courses per semester, health benefits, and a stipend. The assistantships are in great demand and are awarded on a highly competitive basis. Obtaining admission is easier than obtaining admission plus financial aid.
I will be applying for a Teaching/Research Assistantship.
